Short Mood & Feelings Questionnaire (SMFQ)
The Short Mood and Feelings Questionnaire (SMFQ) is a13-item subscale from a longer 33-item questionnaire (the original MFQ). The SMFQ is a brief, easy to administer, self-report measure of childhood and adolescent depression, designed for rapid evaluation of core depressive symptomology or for use in epidemiological studies. The questions asked on the survey are based on the DSM-III criteria for depression; however, this instrument should be used as an indicator of depressive symptoms and not as a diagnostic tool; therefore it does not indicate whether a child or adolescent has a particular disorder. This instrument has a companion version, (i.e., the parent/caregiver-report version), which consists of items assessing the same depressive symptoms by a proxy. Each item is rated on a 3-point Likert scale: "true", "sometimes true", and "not true" with respect to events of the past two weeks.
